An inquiry has found that a mass killing by a British teenager in 2024 could have been prevented if his parents and state agencies had acted on his violence fixation.
A retired judge who led a nine-week inquiry into the attack at a Taylor Swift-themed dance class listed the numerous missed opportunities to intervene in the life of Axel Rudakubana, who fatally stabbed three young girls and injured 10 others in Southport, northwest England.
British Prime Minister Keir Starmer has promised changes to address the systematic failures that led to the tragedy.
