The investigation found that a firefighter on the truck heard a controller warn 'stop, stop, stop' but did not realize the command was directed at his vehicle.

Furthermore, a key runway safety system failed to activate, and the fire trucks involved were not equipped with transponders that could have alerted controllers to the imminent collision.

The controller had cleared the fire truck to cross the active runway just minutes after instructing the jet to land.

The NTSB is now recommending that all school buses and emergency vehicles be equipped with alcohol testing and improved tracking technology to prevent similar tragedies.
