The first phase of the Southport Inquiry has laid bare a series of systemic failures that allowed Axel Rudakubana to carry out a murderous attack at a children's dance workshop.

Inquiry chairman Sir Adrian Fulford identified sixty-seven recommendations for reform, noting that the killer's behavior had been 'unambiguously signposted' for years.

The report criticizes the police, NHS, and social services for a lack of curiosity regarding the suspect's online activities and access to weapons.

Families of the victims are now calling for urgent legislative changes to ensure such a tragedy never recurs.
