Two pilots were killed and thirty-nine people were hospitalized following the March 22 incident on the New York runway.

Investigators found that the fire truck crew heard a command to stop only moments before the impact, suggesting a delay in critical instructions.

The NTSB is also examining why a key runway safety system failed to alert the tower of the impending collision.

The findings have prompted calls for a comprehensive review of emergency response protocols at major international airports.
