Southport Murders Were Preventable
The first phase of the Southport Inquiry report has laid bare a series of catastrophic missed opportunities to stop Axel Rudakubana before he murdered three young girls.
The inquiry found that Rudakubana's parents failed in their 'moral duty' to report his hoarding of weapons and fixation on extreme violence to authorities.
Agencies including the police and social services were also criticized for poor information sharing and a misunderstanding of the killer's autism.
Families of the victims are now calling for urgent legislative reforms to prevent similar tragedies.
