Southport inquiry finds systemic failures

A public inquiry into the 2024 Southport killings has concluded that the tragedy was preventable and resulted from 'catastrophic' failures by state bodies and the killer's parents.

The report found that Axel Rudakubana's escalating violent behavior and fixation on weapons were known but not properly addressed by social services or the police.

Chairman Sir Adrian Fulford criticized a 'merry-go-round of buck-passing' between agencies that failed to recognize the risk Rudakubana posed.

The inquiry issued 67 recommendations to prevent similar future attacks, including legislative changes for individuals planning violence without an underlying ideology.
